Promifen Tablets (Generic Clomiphene Citrate Tablets) belong to the group of medications called Selective Estrogen Receptor Modulator (SERM), are used for induction of ovulation in women with persistent ovulatory dysfunction who desire pregnancy. Promifen tablets can be used to make an irregularly ovulating woman ovulate regularly.

Generic Clomiphene is useful in the treatment of oligomenorrhoea, dysfunctional uterine bleeding, amenorrhoea, post pill menorrhoea, Stein Leventhal syndrome (polycystic ovarian diseases), and Chiari Frommel syndrome. Promifen tablets are also used to treat male infertility. Clomiphene is also used offlabel by bodybuilders during Post Cycle Therapy (PCT). Active Pharmaceutical Ingredient in Promifen

The active ingredient present in Promifen tablets is Clomiphene Citrate. Each tablet of Promifen contains 50 mg Clomiphene Citrate.

Uses of Promifen Tablets

Promifen Tablets  is used in the treatment of:

• Female Infertility
• Lactation Suppression
• Oligospermia  (low sperm counts)
• Ovulation Induction

Clomiphene Citrate (Promifen) stimulates the release of hormones necessary for ovulation to occur.
Clomiphene is used to stimulate ovulation (the release of an egg) when a woman’s ovaries can produce a follicle but hormonal stimulation is deficient.
Promifen causes the pituitary gland to produce more follicle stimulating hormone (FSH) and luteinizing hormone (LH), which, in turn, stimulate the ovaries into producing more eggs and follicles. Impediments to achieving pregnancy must be excluded or adequately treated before you start therapy using Promifen. Clomifene citrate may also be used for the treatment of secondary male hypogonadism.

Promifen Tablets – Dosage

Female infertility
One tablet of Promifen 50 mg  should be administered daily for five days starting on the 5th day of the menstrual cycle. If ovulation does not appear to have occurred, a second course of two tablets of Promifen 50 daily (in a single dose) for five days should be given starting from the fifth day of the next cycle. A third treatment cycle may follow, with dosage regimens that are similar to the second. An interval of 30 days should be maintained between consecutive courses. In amenorrhoeic patients, the regimen is identical, but treatment can be started on any day.

Male infertility
A dose of one tablet of Promifen 25 daily for 25 days followed by a rest period of five days for three months.

What to do in case of a Missed Dose

Promifen Tablets – Contraindications

Promifen tablets   are contraindicated in patients with a known hypersensitivity to Clomiphene Citrate or any inactive ingredient present in Promifen. Promifen tablets are also contraindicated in the following cases:

Liver disease:
Promifen is contraindicated in patients with known liver disease or a history of liver dysfunction.
Hormone-dependent tumours or abnormal uterine bleeding:
Promifen is contraindicated in patients with hormone-dependent tumours or in patients with abnormal uterine bleeding of undetermined origin.
Pregnancy:
Promifen should not be administered during pregnancy. To avoid inadvertent administration during early pregnancy, appropriate tests should be utilised during each treatment cycle to determine whether ovulation occurs. The patient should have a pregnancy test before the next course of Promifen therapy.
Ovarian cyst:
Promifen should not be given in the presence of an ovarian cyst, except polycystic ovary, since further enlargement of the cyst may occur. Patients must be evaluated for the presence of ovarian cyst prior to each course of treatment with Generic Clomiphene.

Promifen and Pregnancy – Success Rate of Promifen

During Clinical trials, 7578 patients were administered Clomiphene Citrate (Promifen). Among them successful therapy characterised by pregnancy occurred in approximately 30% of these patients. A total of of 2635 pregnancies were reported. Data available on 2369 of the cases shows that there were 483 Spontaneous Abortions and 24 Stillbirths. Among the live births there were 1697 Single Births and 165 Multiple Births. The survival rate for Single Births was 98.16% and 83.25% for Multiple Births.

Warnings and Precautions when using Promifen Tablets

Use Generic Clomiphene with caution when driving, operating machinery, or performing other hazardous activities. Clomiphene Citrate may cause blurred vision or other visual side effects during or shortly after therapy. Inform your doctor immediately in case you develop any visual side effects and use caution when performing hazardous activities, especially under conditions of variable lighting.
Clomiphene may increase the likelihood of multiple births. Multiple births may carry additional risk both for the mother and for the fetuses.

Do not take clomiphene  without first consulting your doctor if you;

• might be pregnant;
• have a thyroid problem or another endocrine disorder;
• have undiagnosed vaginal bleeding;
• have endometriosis or endometrial carcinoma;
• have uterine fibroids;
• have liver disease;
• have ovarian cysts or ovarian enlargement not due to polycystic ovarian syndrome; or
• have any other serious or chronic medical illness.

Clomiphene Citrate Side Effects

The possible side effects of Clomiphene Citrate Tablets are listed below.

• Ovarian hyperstimulation, ranging from mild, with enlarged ovaries and abdominal discomfort; to moderate, additionally causing nausea, vomiting, or shortness of breath; to severe and life-threatening.
• Hot flashes.
• Irritability.
• Nausea, abdominal pain.
• Headaches.
• Thick cervical mucus, which sperm cannot travel through. This can be reversed with medicine or bypassed with intrauterine insemination.
• Breast tenderness.
• Blurred vision.
• Hair loss (very rare).

Generic Clomiphene Overdosage

There have been no reports of toxic effects accompanying acute overdosage of Clomiphene Citrate. Signs and symptoms of overdosage as a result of the use of more than the recommended dose during Clomiphene Citrate therapy include nausea, vomiting, vasomotor flushes, visual blurring, spots or flashes, scotomata, ovarian enlargement with pelvic or abdominal pain.

In the event of Generic Clomiphene overdose, appropriate supportive measures should be employed in addition to gastrointestinal decontamination.

Clomiphene Citrate During Pregnancy

Clomiphene Citrate has been classified by the US FDA as Pregnancy Category X. Clomiphene Citrate use in pregnant women is contraindicated as it does not offer any benefit in this population group.

Available human data do not suggest an increased risk for congenital anomalies above the background population risk when used as indicated.
