Alphagra Pocket tablets (Generic Sildenafil Citrate) are used for the treatment of male sexual function problems (erectile dysfunction or inability to sustain a satisfactory erection to complete intercourse). It is also used in the treatment of Pulmonary Arterial Hypertension (PAH). In combination with sexual stimulation, sildenafil works by increasing blood flow to the penis to help a man get and keep an erection.This drug does not protect against sexually transmitted diseases. Alphagra Pocket tablets are available in strength of 100 mg and are supplied in a box of 100 tablets (Box contains 25 blister strips of 4 tablets each).

Active Pharmaceutical Ingredient
The active pharmaceutical ingredient contained in Alphagra Pocket tablets is Sildenafil Citrate. Each film-coated tablet contains 100 mg Sildenafil (as sildenafil Citrate).
Uses of Alphagra Pocket Tablets
Alphagra Pocket (Sildenafil Citrate tablets ) is a treatment for adult men with erectile dysfunction, sometimes known as impotence. This is when a man cannot get, or keep a hard, erect penis suitable for sexual activity.
Alphagra Pocket contains the active substance Sildenafil Citrate which belongs to a group of medications called phosphodiesterase type 5 (PDE 5) inhibitors. It works by helping to relax the blood vessels in your penis, allowing blood to flow into your penis when you get sexually excited. Sildenafil will only help you to get an erection if you are sexually stimulated.
Alphagra Pocket Tablets – Dosage
For most patients, the recommended dose is 50 mg taken, as needed, approximately 1 hour before sexual activity. However, Alphagra Pocket may be taken anywhere from 30 minutes to 4 hours before sexual activity.
Based on effectiveness and toleration, may increase to a maximum of 100 mg or decrease to 25 mg. Alphagra Pocket tablets may be taken with or without food. The maximum recommended dosing frequency is once per day, i.e. 24 hours must elapse befor you take the next dose.
Dosage Adjustment in the Elderly: Since Sildenafil clearance is reduced in elderly patients, a first dose of 25 mg should be considered. Based on efficacy and toleration, the dose may be increased to 50 mg and 100 mg.

Alphagra Pocket Tablets – Contraindications
Alphagra Pocket tablets are contraindicated in patients with a hypersensitivity to Sildenafil Citrate or any inactive ingredient present in Alphagra Pocket tablets. They are also contraindicated in the following cases:
- Administration of Alphagra Pocket to patients using nitric oxide donors, such as organic nitrates or organic nitrites in any form is contraindicated. Alphagra Pocket was shown to potentiate the hypotensive effect of nitrates.
- Severe hepatic impairment (decreased liver function)
- Severe impairment in renal function
- Alphagra Pocket is contraindicated in men for whom sexual intercourse is inadvisable due to cardiovascular risk factors
- Hypotension (low blood pressure)
- Recent stroke or heart attack
- Hereditary degenerative retinal disorders (including genetic disorders of retinal phosphodiesterases)
- Do not use Alphagra Pocket (Sildenafil tablets) in patients who are using a GC stimulator, such as Riociguat. PDE5 inhibitors, including sildenafil tablets, may potentiate the hypotensive effects of GC stimulators.
Storage Instructions for Alphagra Pocket Tablets
Store Alphagra Pocket tablets in a cool dry place where the temperature stays below 30°C. Do not store it or any other medicine in the bathroom or near a sink. Keep away from children or pets.
Warnings and Precautions to be taken when using Alphagra Pocket Tablets
Before using Alphagra Pocket tablets please inform your doctor about all the medicines that you take including no prescription medications, over the counter medicines and herbal remedies.
Also inform your doctor about all your existing medical conditions including but not limited to heart problems such as a heart attack, irregular heartbeat, angina, chest pain, narrowing of the aortic valve or heart failure, heart surgery, stroke, low or high blood pressure, deformed penis, erection lasting 4 hours or more, sickle cell anemia, multiple myeloma, or leukemia, retinitis pigmentosa, temporary severe vision loss, bleeding problems, stomach ulcers, liver or kidney problems. The other warnings and precautions to be taken are:
- Patients should not use Alphagra Pocket tablets if sexual activity is inadvisable due to cardiovascular status
- Patients should seek emergency treatment if an erection lasts for more than 4 hours. Use Alphagra Pocket with caution in patients predisposed to priapism.
- Patients should stop Alphagra Pocket and seek medical care if a sudden loss of vision occurs in one or both eyes, which could be a sign of non arteritic anterior ischemic optic neuropathy (NAION). Alphagra Pocket should be used with caution, and only when the anticipated benefits outweigh the risks, in patients with a history of NAION. Patients with a "crowded" optic disc may also be at an increased risk of NAION.
- Patients should stop using Alphagra Pocket and seek prompt medical attention in the event of sudden decrease or loss of hearing.
- Caution is advised when Alphagra Pocket is co-administered with alpha-blockers or anti-hypertensives. Concomitant use may lead to hypotension.
- Decreased blood pressure, syncope, and prolonged erection may occur at higher sildenafil exposures. In patients taking strong CYP inhibitors, such as ritonavir, sildenafil exposure is increased. Decrease in Alphagra Pocket dosage is recommended.
Driving And Using Machines
Before you perform tasks which may require special attention, wait until you know how you respond to Alphagra Pocket. As transient visual disturbances and dizziness have been reported in some patients taking Sildenafil, particularly at the 100 mg dose, patients should be aware of how they react to Alphagra Pocket before driving or operating machinery, and the doctor should advise accordingly.
Side Effects of Alphagra Pocket Tablets
The most common possible side effects with an incidence rate of 2% or more include headache, flushing, dyspepsia, abnormal vision, nasal congestion, back pain, myalgia, nausea, dizziness and rash.
The following events occurred in less than 2% of patients in Sildenafil Citrate controlled clinical trials; a causal relationship to Sildenafil Citrate tablets is uncertain :
Body as a Whole: face edema, photosensitivity reaction, shock, asthenia, pain, chills, accidental fall, abdominal pain, allergic reaction, chest pain, accidental injury.
Cardiovascular: angina pectoris, AV block, migraine, syncope, tachycardia, palpitation, hypotension, postural hypotension, myocardial ischemia, cerebral thrombosis, cardiac arrest, heart failure, abnormal electrocardiogram, cardiomyopathy.
Digestive: vomiting, glossitis, colitis, dysphagia, gastritis, gastroenteritis, esophagitis, stomatitis, dry mouth, liver function tests abnormal, rectal hemorrhage, gingivitis.
Hemic and Lymphatic: anemia and leukopenia.
Metabolic and Nutritional: thirst, edema, gout, unstable diabetes, hyperglycemia, peripheral edema, hyperuricemia, hypoglycemic reaction, hypernatremia.
Musculoskeletal: arthritis, arthrosis, myalgia, tendon rupture, tenosynovitis, bone pain, myasthenia, synovitis.
Nervous: ataxia, hypertonia, neuralgia, neuropathy, paresthesia, tremor, vertigo, depression, insomnia, somnolence, abnormal dreams, reflexes decreased, hypesthesia.
Respiratory: asthma, dyspnea, laryngitis, pharyngitis, sinusitis, bronchitis, sputum increased, cough increased.
Skin and Appendages: urticaria, herpes simplex, pruritus, sweating, skin ulcer, contact dermatitis, exfoliative dermatitis.
Special Senses: sudden decrease or loss of hearing, mydriasis, conjunctivitis, photophobia, tinnitus, eye pain, ear pain, eye hemorrhage, cataract, dry eyes.
Urogenital: cystitis, nocturia, urinary frequency, breast enlargement, urinary incontinence, abnormal ejaculation, genital edema and anorgasmia.

Sildenafil Citrate Tablets (Alphagra Pocket) Overdosage
In studies with healthy volunteers of single doses up to 800 mg of Sildenafil Citrate tablets, adverse reactions were similar to those seen at lower doses but incidence rates and severities were increased. Survival is reported in a 42-year-old female following an overdose with 2,000 mg sildenafil.
In cases of Alphagra Pocket overdose, standard supportive measures should be adopted as required. Sildenafil blood levels are not clinically useful. Monitor ECG and blood pressure in symptomatic patients. Renal dialysis is not expected to accelerate clearance as Sildenafil is highly bound to plasma proteins and it is not eliminated in the urine.
Sildenafil Citrate Tablets (Alphagra Pocket) During Pregnancy
Sildenafil Citrate (Alphagra Pocket tablets) has been classified by the US FDA as Pregnancy Category B. Alphagra Pocket(Sildenafil Citrate Tablets) is not indicated for use in women.
There are no adequate and well-controlled studies of Sildenafil in pregnant women. No evidence of teratogenicity, embryotoxicity, or fetotoxicity was observed in pregnant rats or rabbits dosed with Sildenafil 200 mg/kg/day during organogenesis, a level that is, on a mg/m2 basis, 32- and 68-times, respectively, the recommended human dose (RHD) of 20 mg three times a day. In a rat pre- and postnatal development study, the no-observed-adverse-effect dose was 30 mg/kg/day (equivalent to 5-times the RHD on a mg/m2 basis).
Sildenafil Citrate Nursing Considerations
Sildenafil Citrate (Alphagra Pocket tablets) is not indicated for use in women.
Limited data indicate that Sildenafil and its active metabolite are present in human milk. There is no information on the effects on the breastfed child, or the effects on milk production.
