Bactrim D.S. is a medicine which is a synthetic antibacterial combination of trimethoprim and sulphamethoxazole. It is used in the treatment of various bacterial infections, such as ear infections, urinary tract infections, bronchitis, traveller’s diarrhoea, and shigellosis. In AIDS patients it is used in treatment and prevention of pneumocystis carinii pneumonia, chancroid, and toxoplasma encephalitis. Bactrim D.S. tablets are available in DS (double strength) tablets. Each tablet contains trimethoprim 160mg and sulphamethoxazole 800mg.

Active Pharmaceutical Ingredient
The active pharmaceutical ingredients contained in Bactrim D.S. are trimethoprim and sulphamethoxazole.
Mechanism of Action
Trimethoprim and sulfamethoxazole work together in a synergistic way by inhibiting successive steps in the folate synthesis in bacterial cells.
Sulfamethoxazole acts like a false-substrate inhibitor of dihydropteroate synthetase. It acts like an analogue of p-aminobenzoic acid (PABA), thus it acts as competitive inhibitors of the enzyme, inhibiting the production of dihydropteroic acid.
Trimethoprim acts by interfering with the action of bacterial dihydrofolate reductase, inhibiting synthesis of folic acid (tetrahydrofolic acid). Bacteria are unable to take up folic acid from the environment (body of host), and thus are dependent on their own indigenous synthesis. Inhibition of the enzyme starves the bacteria of two bases necessary for DNA replication and transcription.

Bactrim DS (Sulfamethoxazole and Trimethoprim Tablets) – Dosage
The dosage of Bactrim DS tablets for various indications is given below:
Dosage in respiratory and urinary tract infections
Adults:
960 mg (maximum up to 2880 mg daily, given in 2 divided doses in severe cases).
Children:
6 weeks to 5 months old 120 mg
6 months to 5 years old 240 mg
6 years to 12 years old 480 mg
Dosage in gastrointestinal tract infections
Adults:
960 mg (maximum up to 2880 mg daily, given in 2 divided doses in severe cases).
Children:
6 weeks to 5 months old 120 mg
6 months to 5 years old 240 mg
6 years to 12 years old 480 mg
Dosage in pneumonia (pneumocystis carinii)
Adults:
120 mg/kg/day in 2-4 divided doses for 14-21 days.
Children above 4 weeks of age:
120 mg/kg daily, given in 2-4 divided doses.
Dosage in prophylaxis for susceptible infections (AIDS)
Adults:
960 mg daily.
Children:
450 mg/meter square (maximum dose 960 mg per day). To be taken for four days in a week with a gap of one day in between.
*Avoid in hepatic (liver) impairment.
*Dose adjustments in renal insufficiency.
Missed Dosage Instructions for Bactrim D.S.
Take the missed dose as soon as you remember. Skip the missed dose, if it is almost time for your next scheduled dose. Do not take extra medicine to make up for the missed dose.
Storage Instructions
Bactrim D.S. (Trimethoprim and Sulfamethoxazole Tablets) should be kept at room temperature of 15 to 30 degree Celsius (59 to 86 degree Fahrenheit).
Warnings and Precautions
Before using Bactrim D.S. (Sulfamethoxazole and Trimethoprim Tablets) please inform your doctor about all the medicines that you take including no prescription medications, over the counter medicines and herbal remedies. You are also advised to get yourself checked for any kind of allergies to sulfa medications, or trimethoprim, or if you have any other allergies before starting tretment with Bactrim D.S..
Before using this medication get yourself checked to rule out any kidney disease, liver disease, blood disorders (such as porphyria, anemia due to folate deficiency), vitamin deficiency (folate or folic acid), asthma, decreased bone marrow function (bone marrow suppression), and metabolic disorder (G6PD deficiency ).
Patients with AIDS are more sensitive to the side effects of this drug, especially skin reactions, fever, and blood disorders.
During pregnancy, this medication should be used only when clearly needed (please consult your physician). It should not be used near the expected delivery date because of possible harm to the unborn baby.
Nursing mothers should not use Bactrim D.S., as it is excreted in breast milk.
Side Effects of Bactrim D.S. Tablets
The possible side effects of Bactrim D.S. tablets(Sulfamethoxazole and Trimethoprim Tablets) are
- Diarrhoea (watery or bloody)
- Fever, chills, swollen glands, body aches (flu symptoms)
- Sores in mouth and throat
- Pale skin
- Feeling light-headed
- rapid heart rate
- Trouble concentrating
- Easy bruising, unusual bleeding (nose, mouth, vagina, or rectum)
- Severe tingling or numbness
- Nausea, upper stomach pain, itching, loss of appetite, dark urine (jaundice like symptoms)
- Urinating less than usual, or not at all
- Hallucinations, seizure (convulsions)
- Low blood sugar
- Skin rashes
- Cough
Drug Interactions
Bactrim D.S. can increase the blood-thinning effects of warfarin, which might lead to bleeding.
It can increase the metabolism (break-down and elimination) of cyclosporine (causing loss of effectiveness of cyclosporine), and can add to the kidney damage caused by cyclosporine.
Hiprex, Urex, Mandelamine causes acidic urine; they should not be used with Bactrim D.S. tablets
Blood levels of phenytoin (Dilantin) may be increased by treatment with Bactrim. This may lead to side effects associated with phenytoin (Dilantin, Dilantin-125) such as dizziness, and reduced attention.
It may also increase blood levels of digoxin (lanoxin), and possibly lead to serious toxic effects.
Anaemia, due to a reduction in folic acid, can occur in persons receiving Bactrim in combination with divalproex, valproic acid (Depakote, Depakote ER, Depakene, Depacon, Stavzor), methotrexate (Rheumatrex, Trexall), pyrimethamine, triamterene, or trimetrexate.
Increased blood levels of potassium may occur when Bactrim D.S. is combined with ACE inhibitors.
