Telmisartan HCT (telmisartan and hydrochlorothiazide) tablets may be administered with other antihypertensive agents.

Active Pharmaceutical Ingredients

The active pharmaceutical ingredients contained in Cresar-H tablets are Telmisartan and Hydochlorothiazide.

Mechanism of Action

Telmisartan is an angiotensin II receptor blocker that shows high affinity for the angiotensin II receptor type 1 (AT1), with a much higher affinity for AT1 than AT2.
Telmisartan blocks the vasoconstrictor and aldosterone secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor in many tissues, such as vascular smooth muscle and the adrenal gland.
In addition to blocking the RAs, Telmisartan also acts as a selective modulator of peroxisome proliferator-activated receptor gamma (PPAR-?), which is a regulator of insulin and glucose metabolism. Telmisartan’s double action protects against the vascular and renal damage, caused by diabetes and cardiovascular disease.
Hydocholorothiazide acts by increasing chloride, sodium, and water excretion by modulating the transport of sodium ions across renal tubular epithelium.

Uses of Cresar-H Tablets

The medicine is prescribed to treat hypertension (high blood pressure). It can be used alone, or in combination with other antihypertensive drugs. It also helps in cardiovascular risk reduction in patients above 55 years of age, who are unable to take ACE inhibitors and, who are at a major risk of cardiovascular events such as MI (myocardial infraction), stroke and death.
Hydrochlorothiazide aids in lowering blood pressure. It is a thiazide diuretic (water pill) that helps prevent your body from absorbing excess amount of salt, which lead to fluid retention.

Cresar-H Tablets – Dosage

The patients whose blood pressure can’t be controlled by Telmisartan mono therapy are prescribed Cresar-H tablets (Telmisartan Hctz).
Initial dose:Telmisartan 40 mg and Hydrocholorothiazide 12.5 mg orally once a day with or without food (prescribed usually by physician in the beginning).
Maximum titration dose recommended: Telmisartan 160 mg and Hydocholrothiazide 25 mg.
*dose adjustments must be considered for patients suffering from renal insufficiency and liver dysfunctions.
*Dose adjustments by physician must be considered for patients undergoing renal dialysis.
Cresar-H tablets may be administered with or without food.

Missed Dosage Instructions for Telmisartan HCTZ Tablets

Take the missed dose of Cresar-H tablets as soon as you remember. If it is almost time for the next dose, skip the missed dose and take your medicine at the next regularly scheduled time. Do not take extra medicine to make up the missed dose.

Telmisartan Hctz (Cresar-H Tablets) – Contraindications

Cresar-H tablets are contraindicated in patients with known hypersensitivity (e.g., anaphylaxis or angioedema) to telmisartan, hydrochlorothiazide, or any other component of this medication.

Storage Instructions for Cresar-H Tablets

Telmisartan HCTZ (Cresar-H Tablets) should be stored at room temperature i.e. 25°C (77°F); excursions permitted to 15°-30°C (59°-86°F). Store away from light and moisture. Do not remove the medication from the blister pack until right before using.

The patient must be tested for any kind of allergies to Telmisartan Hctz before starting the treatment with it.
*The medical history of the patient has to be assessed, especially of kidney disease (such as renal artery stenosis), liver disorders, bile duct blockage, high blood levels of potassium (hyperkalaemia), loss of too much body water (volume depletion, dehydration) and minerals .
*Do not drive, use heavy machinery, or do any activity that requires alertness while initially on Telmisartan Hctz.
*Limit alcoholic beverages, as alcohol use with Telmisartan Hctz may reduce blood pressure.
*Telmisartan Hctz increases potassium levels in body. Dose adjustments are required, if using potassium supplements along with Telmisartan Hctz.
*In Pregnancy usage of Telmisartan Hctz is strictly prohibited, as it may lead to birth defects.
*Nursing mothers should not take Telmisartan Hctz, as it is excreted in breast milk.

Side Effects of Telmisartan HCTZ Tablets

  • Worsening of renal function may occur in patients dependent on renin-angiotensin-aldosterone system.

Telmisartan HCTZ (Cresar-H Tablets) – Drug Interactions

The drug Interactions of Telmisartan Hctz (Cresar tablets) are given below:
Digoxin when co-administered with Telmisartan Hctz. Digoxin levels in plasma are elevated. Dose monitoring of Digoxin is recommended.
Lithium and Telmisartan Hctz should not be taken concomitantly strictly because of the possible interactions.
NSAIDS and Telmisartan Hctz, when administered together, may lead to acute renal failure. Dose monitoring of both the drugs is recommended.
Ramipril, when co administered with Telmisartan Hctz, leads to increase in plasma concentration of Ramipril. Dose monitoring is recommended.
Warfarin concentration may reduce, when administered with Telmisartan Hctz without affecting INR. Therapy monitoring is thus recommended.
Anti-diabetic drugs when co-administered with Telmisartan Hctz, both drugs may require dose adjustments.
Alcohol, barbiturates and narcotics are strictly prohibited when on Telmisartan Hctz, as it may lead to orthostatic hypertension.
