Warfarin tablets (Warf) helps to prevent blood from excessive clotting or forming harmful clots. It is used after a patient has formed a blood clot in a vein or in patients with a clotting disorder. Drug Uses
Warf (Warfarin tablets) is a vitamin K antagonist (acts against vitamin K) that is used for prevention and treatment of venous thrombosis and its extension, pulmonary embolism, prevention and treatment of thromboembolic complications associated with atrial fibrillation and/or cardiac valve replacement and reduction in the risk of death, recurrent myocardial infarction, and thromboembolic events such as stroke or systemic embolization after myocardial infarction.
Warfarin Tablets (Warf) – Dosage
The dosage of Warfarin tablets has to be individualized for each patient, and adjusted based on INR test (International Normalized Ratio, a laboratory test measure of blood coagulation). An INR test is performed to to see how fast your blood clots. Use of a large initial dose may increase the incidence of haemorrhage and other complications, does not offer more rapid protection against thrombi formation.
It is recommended that therapy with Warf tablets be initiated with a dose of 2 mg to 5 mg per day with dosage adjustments based on the results of INR and/or PT ratio (Prothrombin Time (PT) is a blood test that determines the time taken for blood to clot).
Drug Class and Mechanism
Warfarin Sodium and other coumarin anticoagulants act by inhibiting the synthesis of vitamin K dependent coagulation factors. When blood forms a clot it does so by a complex process involving multiple clotting factors that act in sequence. These factors are produced in the liver. The liver uses vitamin K to produce these factors. Warfarin blocks the availability of vitamin K and thus limits the amount of clotting factors produced in the liver. The degree of depression is dependent upon the dosage administered. The goal of therapy with Warfarin Sodium is to decrease blood clot formation but not to prevent clotting completely.
What to do in Case You Miss a Dose of Warfarin Tablets (Warf)
If you forget to take a dose of Warf, take the dose as soon as you remember and then go back to normal dosing schedule. However, if it is almost time for your next dose (eg. within 8 hours of your scheduled dose), skip the missed dose and take your next dose when you were supposed to take it.
Do not try to make up for a missed dose by taking more than one dose per day.
Storage Instructions for Warfarin Tablets (Warf)
Store Warf tablets at controlled room temperature 15°C to 30°C(59°F to 86°F). Do not store Warf in a bathroom or near a sink. Protect from heat, light and moisture. Heat and dampness can ruin some medicines. Dispense in a tight, light-resistant container. Keep Warfarin tablets out of the reach of children and pets.
Warnings and Precautions to be taken when using Warfarin Sodium Tablets
Before using Warfarin tablets please inform your doctor all the medicines that you take including no prescription medications, over the counter medicines and herbal remedies. The other precautions to be taken when using Warfarin tablets are:
- Hemorrhage: Warfarin tablets can cause major or fatal bleeding. Bleeding is more likely to occur within the first month. Risk factors for bleeding include high intensity of anticoagulation (INR >4.0), age greater than or equal to 65, history of highly variable INRs, history of gastrointestinal bleeding, hypertension, cerebrovascular disease, anemia, malignancy, trauma, renal impairment, certain genetic factors, certain concomitant drugs, and long duration of Warfarin Sodium therapy.
- Tissue Necrosis: Necrosis and/or gangrene of skin and other tissues is an uncommon but serious risk (<0.1%). Necrosis may be associated with local thrombosis and usually appears within a few days of the start of Warfarin therapy.
- Systemic Atheroemboli and Cholesterol Microemboli: Anticoagulation therapy with Warf tablets may enhance the release of atheromatous plaque emboli. Systemic atheroemboli and cholesterol microemboli can present with a variety of signs and symptoms depending on the site of embolization. The most commonly involved visceral organs are the kidneys followed by the pancreas, spleen, and liver.
- Heparin-Induced Thrombocytopenia: Do not use Warfarin tablets as initial therapy in patients with heparin-induced thrombocytopenia (HIT) and with heparin-induced thrombocytopenia with thrombosis syndrome (HITTS). Cases of limb ischemia, necrosis, and gangrene have occurred in patients with HIT and HITTS when heparin treatment was discontinued and warf therapy was started or continued.
- Use in Pregnant Women with Mechanical Heart Valves: Warfarin tablets can cause fetal harm when administered to a pregnant woman. While Warfarin is contraindicated during pregnancy, the potential benefits of using Warfarin tablets may outweigh the risks for pregnant women with mechanical heart valves at high risk of thromboembolism. Warfarin exposure during pregnancy causes a recognized pattern of major congenital malformations (warfarin embryopathy and fetotoxicity), fatal fetal hemorrhage, and an increased risk of spontaneous abortion and fetal mortality.
- Females of Reproductive Potential: Warfarin exposure during pregnancy can cause pregnancy loss, birth defects, or fetal death.
Side Effects of Warfarin Tablets (Warf)
The most common possible side effects to Warfarin Sodium tablets are fatal and nonfatal hemorrhage from any tissue or organ, Necrosis of skin and other tissues and Systemic atheroemboli and cholesterol microemboli. Other possible side effects may include hypersensitivity/allergic reactions (including urticaria and anaphylactic reactions), vasculitis, hepatitis, elevated liver enzymes, nausea, vomiting, diarrhea, taste perversion, abdominal pain, flatulence, bloating, rash, dermatitis (including bullous eruptions), pruritus, alopecia, tracheal or tracheobronchial calcification.
Warfarin Tablets (Warf) – Contraindications
