The active pharmaceutical ingredient contained in Cresar tablets is Telmisartan. It blocks angiotensin II receptors in the body.

Uses of Telmisartan Tablets

Telmisartan (Cresar tablets) is prescribed to treat hypertension (high blood pressure). It can be used alone, or in combination with other antihypertensive drugs. It also helps in cardiovascular risk reduction in patients above 55 years of age, who are unable to take ACE inhibitors, and who are at a major risk of cardiovascular events such as MI (myocardial infraction), stroke and death.

Telmisartan (Cresar Tablets) – Dosage

Initial dose: 40 mg orally once a day with or without food (prescribed usually by physician in the beginning). Maintain adequate intravascular volume (hydration).
Maintenance dose: 20 mg to 80 mg orally once a day.
Dose for cardiovascular risk reduction in patients unable to take ACE inhibitors: 80 mg orally once a day with or without food.
In cardiovascular risk reduction patients taking Telmisartan, blood pressure should be regularly monitored, and dose adjustments are recommended. Dose adjustments must be considered for patients suffering from renal insufficiency and liver dysfunctions. Dose adjustments by physician must be considered for patients on Telmisartan undergoing renal dialysis.

Mechanism of Action

Telmisartan is an angiotensin II receptor blocker that shows high affinity for the angiotensin II receptor type 1 (AT1), with a much higher affinity for AT1 than AT2. In addition to blocking the RAs, Telmisartan also acts as a selective modulator of peroxisome proliferator-activated receptor gamma (PPAR-?), which is a regulator of insulin and glucose metabolism. Telmisartan’s double action protects against the vascular and renal damage, caused by diabetes and cardiovascular disease.

Missed Dosage Instructions for Telmisartan Tablets

Take the missed dose of Cresar Tablets as soon as you remember. Skip the missed dose, if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

Storage Instructions for Telmisartan Tablets

Telmisartan (Cresar Tablets) should be stored at room temperature away from light and moisture. Do not remove the medication from the blister pack until right before using.

Before using Telmisartan tablets please inform your doctor about all the medicines that you take including no prescription medications, over the counter medicines and herbal remedies.
Before starting the treatment with Telmisartan (Cresar tablets), patient must be tested for any kind of allergies to it.
Medical history must be assessed, especially of kidney disease (such as renal artery stenosis), liver disorders, bile duct blockage, high blood levels of potassium (hyperkalaemia), loss of too much body water (volume depletion, dehydration) and minerals.
Do not drive, use heavy machinery, or do any activity that requires alertness while initially on Telmisartan.
Limit alcoholic beverages, as alcohol use with Telmisartan may reduce blood pressure.
Telmisartan increases potassium levels in body. Dose adjustments are required if using potassium supplements along with Cresar tablets (Telmisartan).
Use of Cresar tablets (Telmisartan) during pregnancy is strictly prohibited, as it may lead to birth defects in the baby.
Nursing mothers should not take Telmisartan, as it is excreted in breast milk.

Side Effects of Cresar Tablets

The possible Side Effects of Telmisartan (Cresar tablets) are

  • Worsening of renal function may occur in patients dependent on renin-angiotensin-aldosterone system.
  • Cardiovascular: Intermittent claudication, chest pain, hypertension, peripheral oedema.
  • Central nervous system: dizziness, fatigue, headache.
  • Dermatologic: skin ulcer.
  • Gastrointestinal: diarrhoea, abdominal pain, dyspepsia and nausea.
  • Genitourinary: urinary tract infection.
  • Neuromuscular & skeletal: back pain, myalgia.
  • Respiratory: upper respiratory infection, sinusitis, cough, and pharyngitis.

Cresar Tablets – Drug Interactions

The drug interactions of Telmisartan (Cresar tablets) are given below:
ACE Inhibitors: Telmisartan may enhance the adverse and toxic effect of ACE Inhibitors. Thus dosage needs to be monitored.
Alfuzosin: May enhance the hypotensive effect of Telmisartan. Thus dosage needs to be monitored.
Aliskiren: May enhance the hyperkalemic effect of Telmisartan. Thus a dose modification by the physician is recommended.
Amifostine: Telmisartan may enhance the hypotensive effect of Amifostine. When amifostine is used at chemotherapy doses, Telmisartan should be withheld for 24 hours prior to amifostine administration.
Antihypertensives: Other antihypertensives given with telmisartan , enhance the hypotensive effect considerably. Thus dose monitoring is emphasized.
Cardiac Glycosides: Telmisartan may increase the serum concentration of Cardiac Glycosides. Monitor therapy when used together.
Diazoxide: May enhance the hypotensive effect of Telmisartan. The dosage needs to be monitored.
Eplerenone: May enhance the hyperkalemic effect of Telmisartan. Dosage needs to be monitored.
Herbs (Hypertensive Properties): May diminish the antihypertensive effect of Telmisartan. Thus herbs should be avoided.
Lithium: Telmisartan may increase the serum concentration of Lithium. Thus Lithium dosage reductions are needed following the simultaneous administration with Telmisartan.
Methylphenidate: May diminish the antihypertensive effect of Telmisartan. Thus dose modifications are recommended.
Non-steroidal anti-inflammatory agents: Telmisartan may enhance the adverse and toxic effect of non-steroidal anti-inflammatory agents: The combination may result in a significant decrease in kidney function. Non-steroidal anti-inflammatory agents may diminish the therapeutic effect of Telmisartan. Thus dose modifications need to be considered.
Pentoxifylline: May enhance the hypotensive effect of Telmisartan. Thus dose modification is necessary.
Phosphodiesterase 5 Inhibitors: May enhance the antihypertensive effect of Telmisartan. Thus dose modification is necessary.
Potassium Salts: May enhance the hyperkalemic effect of Telmisartan. Dose modification is required.
Potassium-Sparing Diuretics: Telmisartan may enhance the hyperkalemic effect of Potassium-Sparing Diuretics. Dose must be monitored therefore.
Prostacyclin Analogues: May enhance the hypotensive effect of Telmisartan. Dose modification is recommended therefore.
Ramipril: Telmisartan may increase the serum concentration of Ramipril. Concentrations of the active metabolite, ramiprilat, may also be increased. Monitor when administered together.
Rituximab: Telmisartan may enhance the hypotensive effect of Rituximab. Consider therapy modification.
Sodium Phosphates: Telmisartan may enhance the nephrotoxic effect of Sodium Phosphates. The risk of acute phosphate nephropathy may be enhanced. Consider avoiding administering together.
Tolvaptan: May enhance the hyperkalemic effect of Telmisartan. Consider monitoring therapy when administered together.
Trimethoprim: May enhance the hyperkalemic effect of Telmisartan. Therapy needs to be monitored.
Yohimbine: May diminish the antihypertensive effect of Telmisartan. Dose modification in such case is needed.
