The active pharmaceutical ingredients contained in Ibugesic Plus Tablets (Generic Ibuprofen and Paracetamol) are Ibuprofen and Paracetamol (Acetaminophen).Each uncoated Ibugesic Plus tablet contains 400 mg of Ibuprofen, IP and 325 mg of Paracetamol, IP.

Drug Uses

Ibugesic Plus Tablets are used to treat pain caused by many conditions such as headache, muscle aches, arthritis , backache, toothaches, and menstrual cramps. Take this medicine for the relief of mild to moderate pain. Your doctor may prescribe Ibugesic Plus for the treatment of primary dysmenorrhea.

Ibugesic Plus Tablets – Dosage

Ibugesic Plus must not be used for more than 3 days at a time. The dosage varies as per the condition being treated. If Ibugesic Plus tablets are required in adults for more than 3 days in the case of fever or for more than 4 days for the treatment of pain or if the symptoms worsen the patient is advised to consult a physician.

Adults and adolescents weighing from 40 kg body weight (aged 12 years and above): Initial
dose, 1 tablet of Ibugesic Plus. If necessary, additional doses of Ibugesic Plus tablet can be
taken. The respective dosing interval should be chosen in line with the observed symptoms
and the maximum recommended daily dose. It should not be below 6 hours. A total dose of 3
tablets of Ibugesic Plus should not be exceeded in any 24-hour period.
For short-term use only.
Adults: The maximum adult dose is 1 tablet 3 times a day.
Consult your doctor for exact dosing instructions. If you experience gastrointestinal toxicity, take Ibugesic Plus Tablets with meals or milk.

Drug Class and Mechanism

Ibuprofen belongs to a class of drugs called non-steroidal anti-inflammatory drugs (NSAIDs). Other members of this class include naproxen (Aleve), indomethacin (Indocin), nabumetone (Relafen) and several others. These drugs are used for the management of mild to moderate pain, fever, and inflammation. Prostaglandins are chemicals that are made by the body and are responsible for causing pain, fever and inflammation. Ibuprofen blocks the enzyme that makes prostaglandins (cyclooxygenase), resulting in lower levels of prostaglandins. As a consequence, inflammation, pain and fever are reduced.

Paracetamol (Acetaminophen) is a centrally acting analgesic and antipyretic with minimal anti-inflammatory properties.
Paracetamol may act predominantly by inhibiting prostaglandin synthesis in the central nervous system and, to a lesser extent, through a peripheral action by blocking pain-impulse generation for analgesic action.

For causing Antipyretic effect, Paracetamol act centrally on the hypothalamic heat-regulating center to produce peripheral vasodilatation resulting in increase blood flow through the skin, sweating and heat loss.

Missed Dose

If you miss a dose, use it as soon as you remember. If it is near the time for the next dose, skip the missed dose and resume your usual dosing schedule. Do not double the dose to catch up.

Storage Instructions for Ibugesic Plus Tablets

Store Ibugesic Plus Tablets at controlled room temperature 20 to 25 degrees C (68 to 77 degrees F).

Warnings and Precautions to be taken when using Ibugesic Tablets

You should not take Ibugesic Plus Tablets if you have previously exhibited hypersensitivity to the drug, or have the syndrome of nasal polyps, angioedema and bronchospastic reactivity to aspirin or other nonsteroidal anti-inflammatory agents. Serious gastrointestinal toxicity such as bleeding, ulceration, and perforation, can occur at any time, with or without warning symptoms.

This product contains Paracetamol (Acetaminophen). Severe liver damage may occur if you take
more than 3 tablets in 24 hours, which is the maximum daily amount. DO NOT USE this medication with with other drugs containing Paracetamol (Acetaminophen). Do not consume 3 or more alcoholic drinks every day while using this product.

Contraindications

Ibugesic is contraindicated for use in patients which have one or more of the below mentioned conditions :

  • A known hypersensitivity or idiosyncratic reaction to Paracetamol, Ibuprofen or any of the other ingredients in the product.
  • A known hypersensitivity to Aspirin and other NSAIDs
  • Asthma that is aspirin or NSAID sensitive
  • In patients with a history of, or an existing gastrointestinal ulceration/ perforation or
    bleeding, including that associated with NSAIDs.
  • In patients with severe hepatocellular insufficiency, severe renal failure or severe
    heart failure.
  • Active, or history of recurrent or existing peptic ulcer/haemorrhages (two or more
    distinct episodes of proven ulceration or bleeding).
  • cerebrovascular or other active bleeding
  • Adolescents under 40 kg body weight and children below 12 years of age.
  • Patients with severe dehydration (caused by vomiting, diarrhoea or insufficient fluid
    intake).
  • In patients with active alcoholism as chronic excessive alcohol ingestion may predispose patients to hepatotoxicity (due to the paracetamol component).
  • During the third trimester of pregnancy.

This medicine should not be taken with other products containing paracetamol, ibuprofen, acetylsalicylic acid, salicylates or with any other anti-inflammatory medicines unless under a doctor’s instruction.

Possible Side Effects of Ibugesic Plus Tablets

The most frequent type of side effect that you may experience with Ibuprofen is gastrointestinal.
The most common Ibuprofen side effects reported in adult patients were nausea, flatulence, vomiting, headache, hemorrhage and dizziness.

The most common Ibuprofen side effects reported in pediatric patients were vomiting, nausea, anemia and headache.

Other side effects that have been reported include Blurred and/or diminished vision, scotomata, and/or changes in color vision.

The most common adverse reactions in patients treated with Paracetamol (Acetaminophen) were nausea, vomiting, headache, and insomnia in adult patients and nausea, vomiting, constipation, pruritus, agitation, and atelectasis in pediatric patients.

More Information Regarding Ibugesic Plus Use

Contact your doctor if you experience blood in vomit or bloody, black, or tarry stools. These symptoms could indicate damage to the stomach or intestines, which could be dangerous.
If you experience any of the following serious side effects, stop taking ibuprofen and seek medical attention or contact your doctor immediately:

  • an allergic reaction (difficulty breathing; closing of the throat; swelling of the lips, tongue, or face; or hives);
  • muscle cramps, numbness, or tingling;
  • ulcers (open sores) in the mouth;
  • rapid weight gain (fluid retention);

  • decreased hearing or ringing in the ears;
  • yellowing of the skin and eyes (jaundice); or

  • dizziness or headache;
  • nausea, gaseousness, diarrhea, or constipation;
  • depression;
  • fatigue or weakness;
  • dry mouth; or
  • irregular menstrual periods.

Side effects other than those listed here may also occur. Talk to your doctor about any side effect that seems unusual or that is especially bothersome.

Effects On Ability To Drive And Use Machines

Undesirable effects such as dizziness, drowsiness, fatigue and visual disturbances are possible after taking NSAIDs. If affected patients should not drive or operate machinery.

Ibugesic Plus Overdosage

Ibuprofen: The symptoms of overdose can include CNS-related symptoms such as headache, dizziness, light-headedness and unconsciousness (also myoclonic convulsions in children), abdominal pain, nausea, vomiting, GI bleeding and hepatic and renal dysfunction, hypotension, respiratory depression and cyanosis. Ibuprofen overdose may cause metabolic acidosis.

In cases of acute overdose, the stomach should be emptied by vomiting or lavage, though little drug will likely be recovered if more than an hour has elapsed since ingestion.
Since the medication is acidic and is excreted in the urine, it is theoretically beneficial to administer alkali and induce diuresis. In addition to supportive measures, the use of oral activated charcoal may help to reduce the absorption and reabsorption of ibuprofen tablets.

Paracetamol: Liver injury and even failure can occur following Paracetamol (Acetaminophen) overdose. Liver damage is possible in adults who have taken 10 g or more of Paracetamol. Ingestion of 5 g or more of paracetamol may lead to liver damage if the patient has one or more of the risk factors.
Symptoms of paracetamol overdose in the first 24 hours are pallor, nausea, vomiting, anorexia and abdominal pain. Liver damage may become apparent 12 to 48 hours after ingestion. Abnormalities of glucose metabolism and metabolic acidosis may occur.
In severe poisoning, hepatic failure may proceed to encephalopathy, coma and death. Acute renal failure with acute tubular necrosis may develop in the absence of severe liver damage. Cardiac arrhythmias have been reported.

Immediate treatment is essential in the management of paracetamol overdose. Despite a lack of significant early symptoms, patients should be referred to a hospital urgently for immediate medical attention. Symptoms may be limited to nausea or vomiting and may not reflect the severity of overdose or the risk of organ damage. Management should be in accordance with established treatment guidelines.

Ibugesic Plus During Pregnancy

Ibuprofen has been classified by the FDA as Pregnancy Category C. Reproductive studies conducted in rats and rabbits have not demonstrated evidence of developmental abnormalities.

However, animal reproduction studies are not always predictive of human response. There are no adequate and well-controlled studies in pregnant women.

Paracetamol (also known as Acetaminophen or APAP) is classified by the US FDA as Pregnancy Category C. Use of this medicine during pregnancy should be avoided unless advised by a doctor. This includes maternal use during labor because of the potential of respiratory depression in the neonate.

The safety of Paracetamol during pregnancy has not been established relative to possible adverse effects on foetal development.
Ibugesic Plus should be used in pregnancy only if the potential benefit justifies the potential risk to the fetus. Do not use this medicine without the advice of your doctor if you are pregnant.

Ibugesic Plus During Breastfeeding

Paracetamol: Paracetamol (also known as Acetaminophen or APAP) is excreted in breast milk but not in a clinically significant amount, and available published data do not contraindicate breastfeeding.
Therefore, it is not necessary to interrupt breastfeeding for short-term treatment with the recommended dose of this product.

Ibuprofen: Studies have demonstrated that following oral administration of Ibuprofen 400 mg, the level of medicine that appeared in breast milk was below detection levels of 1 µg/mL. The amount of ibuprofen to which an infant would be exposed through this source was considered negligible.
However, since the absolute safety of ibuprofen ingested under these circumstances has not been determined, nursing mothers should be advised to consult a physician before using Ibuprofen.

Thus, Ibugesic Plus Tablets should be used with caution in nursing mothers.
