Naltrexone Tablets (Nodict by Sun Pharma) - Product Information

Nodict (Naltrexone Hydrochloride tablets) is used for the treatment of alcohol dependence and for stopping or markedly reducing the effects of all opioids. The main use of Naltrexone (Nodict tablets) is for the treatment of alcohol dependence, since Naltrexone is found to be effective to decrease heavy drinking.
Treatment with Naltrexone Generic (Nodict tablets) should be part of a comprehensive management program for alcohol and opioid addictions that includes psychosocial support. Nodict tablets are manufactured by Sun Pharmaceutical Industries Ltd., India in the strengths of 50 mg.

Active Ingredient present in Nodict Tablets

The active ingredient present in Nodict tablets is Naltrexone Hydrochloride. Each film-coated tablet of Nodict contains 50 mg of Naltrexone Hydrochloride.



Naltrexone Mechanism of Action

The mechanism of action of Naltrexone Hydrochloride in alcoholism is not fully understood. Naltrexone Hydrochloride, an opioid receptor antagonist, quickly binds to such receptors and blocks the effects of endogenous opioids. It has been proved that Opioid antagonists reduce alcohol consumption by animals, and Naltrexone Hydrochloride has been shown to reduce alcohol consumption in clinical studies.



Generic Naltrexone (Nodict Tablets) - Dosage

Inorder to reduce the risk of precipitated withdrawal in patients dependent on opioids, or increase of a preexisting subclinical withdrawal syndrome, opioid-dependent patients, including those being treated for alcohol dependence, should be opioid-free (including tramadol) for an interval of a minimum of 7 to 10 days before starting Generic Naltrexone treatment.

Treatment of Alcoholism :
The recommended dosage of Nodict tablets for alcohol dependence to support abstinence is 50 mg per day (1 tablet). Since Generic Naltrexone treatment is an adjunctive treatment of alcoholism and the full recovery process from alcohol dependence is individually variable, no standard duration of treatment can be specified. Generic Naltrexone tablets should be considered as only one of several factors that will determine the success of treatment of alcoholism. An initial period of three months should be considered. However, prolonged administration of Nodict tablets may be necessary.

Treatment of Opioid Dependence :
Treatment of Opioid dependence should be initiated with a dose of 25 mg (half a tablet). This can be followed by the usual dose of one tablet of 50 mg per day.



Generic Naltrexone (Nodict Tablets) Contraindications

Generic Naltrexone (Nodict Tablets) is contraindicated in the following cases:

  • Patients with a hypersensitivity to Naltrexone Hydrochloride or any inactive ingredient present in Nodict tablets.
  • Severe renal impairment
  • Severe hepatic impairment
  • Acute hepatitis
  • Opioid addicted patients with a current abuse of opioids since an acute withdrawal syndrome may ensue.
  • Any patient who has failed the Naloxone provocation test or positive screening result for opioids.
  • Patients receiving opioid analgesics or any medication containing opioid.
  • Patients which are currently maintained on opiate agonists (e.g., methadone) or partial agonists (e.g., buprenorphine).
  • It is not known whether there is any cross-sensitivity with naloxone or the phenanthrene containing opioids

 

Naltrexone Side Effects

A small fraction of patients undergoing treatment for in alcoholism with Nodict may experience an opioid withdrawal-like symptom complex consisting of tearfulness, mild nausea, abdominal cramps, restlessness, bone or joint pain, myalgia, and nasal symptoms.

The following side effects were reported in 2% or more of the patients during a study conducted on patients being treated with Naltrexone tablets for Alcoholism : nausea (10%), headache (7%), dizziness (4%), nervousness (4%), fatigue (4%), insomnia (3%), vomiting (3%), anxiety (2%) and somnolence (2%).

In case of patients undergoing treatment for Opioid Addiction the following side effects have been reported during Naltrexone clinical trials in opioid addiction at an incidence rate of more than 10%: Difficulty sleeping, anxiety, nervousness, abdominal pain/cramps, nausea and/or vomiting, low energy, joint and muscle pain, and headache. The other side effects occuring in less than 10% of patients were Loss of appetite, diarrhea, constipation, increased thirst, increased energy, feeling down, irritability, dizziness, skin rash, delayed ejaculation, decreased potency, and chills.



Warnings and Precautions

The following are the warnings and precutions to be taken when undergoing treatment with Generic Naltrexone (Nodict Tablets):

  • Administartion of high doses of opioid along with Naltrexone treatment, can lead to life-threatening opioid poisoning from respiratory and circulatory impairment. Severe opioid withdrawal syndromes caused by the accidental ingestion of Naltrexone tablets have also occured in opioid-dependent individuals.
  • Naltrexone treatment can be started only when the opioid has been discontinued for a sufficiently long period (about 5 to 7 days for heroin and at least 10 days for methadone).
  • During treatment with Naltrexone tablets, painful conditions should be treated with non-opioid analgesia only.
  • Naltrexone tablets have the ability to cause hepatocellular injury when given in excessive doses.
  • Naltrexone tablets are contraindicated in acute hepatitis or liver failure, and its use in patients with active liver disease must be carefully considered in light of its hepatotoxic effects.
  • Since Naltrexone is a potent antagonist with a very long duration of pharmacologic effect (24 to 72 hours), the blockade produced by Naltrexone can be overcome. This is useful in the case of patients who may require analgesia, but poses a potential threat to individuals who attempt, on their own, to overcome the blockade by administering large amounts of exogenous opioids.
  • For emergency situations which may take place in patients receiving fully blocking doses of naltrexone, a suggested plan of management is regional analgesia, conscious sedation with a benzodiazepine, use of non-opioid analgesics or general anesthesia.
  • Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption should not be administered Naltrexone tablets.
  • Naltrexone as well as its primary metabolite are excreted mainly in the urine, and caution is recommended in administering the drug to patients with renal impairment.
  • Precautions should be taken when Naltrexone HCl tablets are administered to patients with liver disease.
  • The possibility of suicide is known to be increased in patients with substance abuse with or without depression. The risk of suicide is not decreased by treatment with Naltrexone tablets.

 

Nodict Tablets - Storage and Handling

Nodict tablets should be stored at temperatures below 25° C. Keep out of reach of children and pets.



Ability to Drive And Use Machines

Naltrexone tablets can impair the mental and/or physical abilities required for performance of potentially hazardous tasks such as driving a car or operating machinery. Patients are advised not to perform such activities during treatment.



Naltrexone During Pregnancy

Naltrexone tablets have been classified by the US FDA as pregnancy Category C. There are no adequate and well controlled studies conducted on Naltrexone Hydrochloride use in pregnancy. Data from animal studies suggests that Naltrexone has been observed to increase the incidence of early fetal loss. Generic Naltrexone tablets should be used during pregnancy when, in the judgement of the attending doctor the potential benefit justifies the potential risk to the fetus.



Naltrexone Tablets During Breastfeeding

Data from animal studies has shown that Naltrexone was excreted in the milk of lactating rats that were dosed orally with Naltrexone. It is unknown whether Naltrexone is excreted in human breast milk. Due to the well known fact that many medicines are excreted in human milk, breast feeding when Generic Naltrexone tablets are administered is not recommended.
