Mechanism of Action
Celecoxib is a nonsteroidal anti-inflammatory drug that exhibits anti-inflammatory, analgesic, and antipyretic activities in animal models. The mechanism of action of Celecoxib is believed to be due to inhibition of prostaglandin synthesis, primarily via inhibition of cyclooxygenase-2 (COX-2), and at therapeutic concentrations in humans, Celecoxib does not inhibit the cyclooxygenase-1 (COX-1) isoenzyme. This pharmacologic activity gives celecoxib its analgesic, anti-inflammatory, and antipyretic effects. In animal colon tumor models, celecoxib reduced the incidence and multiplicity of tumors.

Cobix (Generic Celecoxib Capsules) – Dosage
Use the lowest effective Cobix dosage for shortest duration consistent with individual patient treatment goals. The dosage for the various indications is given below:
Osteoarthritis (OA): 200 mg once daily or 100 mg twice daily.
Rheumatoid Arthritis (RA): 100 mg to 200 mg twice daily.
Juvenile Rheumatoid Arthritis (JRA) in patients 2 years and older: 50 mg twice daily in patients 10-25 kg. 100 mg twice daily in patients more than 25 kg.
Ankylosing Spondylitis (AS): 200 mg once daily single dose or 100 mg twice daily. If no effect is observed after 6 weeks, a trial of 400 mg (single or divided doses) may be of benefit.
Acute Pain (AP)and Primary Dysmenorrhea (PD):400 mg initially, followed by 200 mg dose if needed on first day. On subsequent days, 200 mg twice daily as needed.
Hepatic Impairment: Reduce daily dose by 50% in patients with moderate hepatic impairment (Child-Pugh Class B).Consider a dose reduction by 50% (or alternative management for JRA) in patients who are known or suspected to be CYP2C9 poor metabolizers.
Missed Dosage Instructions for Celecoxib Capsules
If you miss a dose of Cobix capsules, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.
Contraindications
Cobix is contraindicated in persons with a hypersensitivity (allergy) to Celecoxib, sulfonamides or any of the other ingredients of this medicine.
Cobix is also contraindicated in persons with a history of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs. Aslo in the setting of CABG surgery.
Storage Instructions

Warnings and Precautions to be taken when using Celecoxib Capsules
Before using Cobix Capsules (Generic Celecoxib Capsules) please inform your doctor about all the medicines that you take including no prescription medications, over the counter medicines and herbal remedies.
Hepatotoxicity: Inform patients of warning signs and symptoms of hepatotoxicity. Discontinue if abnormal liver tests persist or worsen or if clinical signs and symptoms of liver disease develop.
Hypertension: Patients taking some antihypertensive medications may have impaired response to these therapies when taking NSAIDs. Monitor blood pressure.
Heart Failure and Edema: Avoid use of Cobix Capsules in patients with severe heart failure unless benefits are expected to outweigh risk of worsening heart failure.
Renal Toxicity: Monitor renal function in patients with renal or hepatic impairment, heart failure, dehydration, or hypovolemia. Avoid use of Cobix Capsules in patients with advanced renal disease unless benefits are expected to outweigh risk of worsening renal function.
Anaphylactic Reactions: Seek emergency help if an anaphylactic reaction occurs.
Exacerbation of Asthma Related to Aspirin Sensitivity: Cobix Capsules is contraindicated in patients with aspirin-sensitive asthma. Monitor patients with preexisting asthma (without aspirin sensitivity).
Serious Skin Reactions: Discontinue Cobix Capsules at first appearance of skin rash or other signs of hypersensitivity.
Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS): Discontinue and evaluate clinically.
Fetal Toxicity: Limit use of NSAIDs, including Cobix Capsules, between about 20 to 30 weeks in pregnancy due to the risk of oligohydramnios/fetal renal dysfunction. Avoid use of NSAIDs in women at about 30 weeks gestation and later in pregnancy due to the risks of oligohydramnios/fetal renal dysfunction and premature closure of the fetal ductus arteriosus.
Hematologic Toxicity: Monitor hemoglobin or hematocrit in patients with any signs or symptoms of anemia.
Side Effects of Cobix Capsules
The Most common Side Effects of Celecoxib capsules (Cobix capsules) during arthritis trials with an incidence rate of 2% and greater and more than placebo are: abdominal pain, diarrhea, dyspepsia, flatulence, peripheral edema, accidental injury, dizziness, pharyngitis, rhinitis, sinusitis, upper respiratory tract infection, rash.
Cobix Capsules – Drug Interactions

Concomitant use of Cobix Capsules and warfarin may result in increased risk of bleeding complications.
Concomitant use of Cobix Capsules increases lithium plasma levels.
Concomitant use of Cobix Capsules may reduce the antihypertensive effect of ACE Inhibitors and angiotensin II antaonists
Use caution with drugs known to inhibit P450 2C9 or metabolized by 2D6 due to the potential for increased plasma levels
Cobix Capsules Overdosage
Symptoms following acute NSAID overdosages have been typically limited to lethargy, drowsiness, nausea, vomiting, and epigastric pain, which have been generally reversible with supportive care. Gastrointestinal bleeding has occurred. Hypertension, acute renal failure, respiratory depression, and coma have occurred, but were rare.
No overdoses of Celecoxib capsules (Cobix capsules) were reported during clinical trials. Doses up to 2400 mg/day for up to 10 days in 12 patients did not result in serious toxicity. No information is available regarding the removal of Celecoxib by hemodialysis, but based on its high degree of plasma protein binding (greater than 97%) dialysis is unlikely to be useful in overdose.
Manage patients with symptomatic and supportive care following an NSAID overdosage. There are no specific antidotes. Consider emesis and/or activated charcoal (60 to 100 grams in adults, 1 to 2 grams per kg of body weight in pediatric patients) and/or osmotic cathartic in symptomatic patients seen within four hours of ingestion or in patients with a large overdosage (5 to 10 times the recommended dosage). Forced diuresis, alkalinization of urine, hemodialysis, or hemoperfusion may not be useful due to high protein binding.
Cobix Capsules During Pregnancy
Celecoxib (Cobix capsules) is classified by the US FDA as Pregnancy Category C prior to 30 weeks gestation and Pregnancy Category D starting at 30 weeks gestation. It is not known whether is excreted in human milk.
Use of NSAIDs, including Celecoxib (Cobix capsules), can cause premature closure of the fetal ductus arteriosus and fetal renal dysfunction leading to oligohydramnios and, in some cases, neonatal renal impairment. Because of these risks, limit dose and duration of Cobix capsules use between about 20 and 30 weeks of gestation and avoid Cobix capsules use at about 30 weeks of gestation and later in pregnancy (see Clinical Considerations, Data).
Use of NSAIDs, including Cobix capsules, at about 30 weeks gestation or later in pregnancy increases the risk of premature closure of the fetal ductus arteriosus.
